Job Details: TRANSPLANT COORDINATOR

 
Description: The Transplant Nurse Coordinator is responsible for coordinating the care of solid organ (heart, lung, heart-lung, liver, kidney, pancreas, and multi-organ) transplant patients through out the phases of the transplant continuum.
The role may include facilitating referrals for evaluation, assuring that transplant patient�s needs are met during the inpatient stay, and for the long term follow-up after discharge from the transplant admission. In this role s/he will be responsible for educating patients, family and other providers about the intricacies of organ transplantation. The ability to communicate to a diverse audience, synthesize and apply advanced clinical knowledge, be flexible, and collaborate in an organized manner is essential to the role. Responsibilities also include on-call coverage to meet patient needs beyond the daily business hours. As a member of the multidisciplinary transplant team, the Transplant Nurse Coordinator plays an integral role in achieving safe, high quality patient outcomes through teaching, research, and leadership.

Education
BSN required; Masters Degree in Nursing preferred.

Experience
5 years nursing experience with at least two years in transplant, critical care, disease specific OR minimum of 3 years clinical nursing experience in transplant, critical care, disease specific. Case management or clinical research experiences also a positive.

Licensure or Certifications
Current North Carolina RN licensure required.
