Description:
Immediate opening for a Clinical Program Coordinator
who will be responsible for the planning and coordination
of all aspects of care for the Post-Kidney Transplant
patients' evaluation throughout the continuation of
services to follow up care.
Serves as the primary
coordinator for the interdisciplinary team, including
physicians, nurses, social workers, dietitians, pharmacists,
financial analysts, case managers and support staff.
Develops, coordinates, implements, and evaluates educational
programs/projects for patients, personnel and the community.
Collects/documents data to meet clinical, financial,
and regulatory requirements. Participates in the development
of guidelines, protocols, pathways and other performance
improvement activities. Participates in research activities.
Serves as a patient advocate throughout the transplant/surgical
process. Represents the Medical Center at regional
and national meetings/conferences.
Requirements
include valid CA RN license, BLS, BSN, and a minimum
of 2 years acute hospital experience, ICU experience
is preferred. A full command (verbal and written) of
the English language is a must! A demonstrated commitment
to customer service and an ability to meet the needs
and expectations of patients and health care colleagues
is also required.
