In
Care Coordination, the patient is at the center of
everything we do. Our goal is to give the right care
at the right time to every patient we serve. The Care
Manager assists the patient throughout their hospital
experience to insure the right care is provided at
the right time. In this important role, you will analyze
clinical information and make recommendations regarding
the appropriateness of care, collaborating with patients,
family, and caregivers to organize and facilitate care
consistent with best practice.
This position
requires a Bachelor's degree in Nursing, at least three
years RN experience in an acute care hospital, and
a current Washington State RN license.
