POSITION PURPOSE Coordinate the Diabetes Education program and integrate administrative and clinical aspects of Diabetes care to provide quality medical Diabetes Education and Nutrition Therapy Inpatient and Outpatient care. Participates in patient care and hospital-wide activities to achieve department and organizational goals. All job duties are performed in a manner that demonstrates the KGH values of caring, respect and professional excellence toward patients, families, volunteers, physicians, and co-workers. POSITION REQUIREMENTS 1. Education: Registered Nurse degree from an accredited nursing program or Bachelors degree from an accredited college or university in Foods and Nutrition or Dietetics. 2. Certification: Certified Diabetes Educator or have at least 24 hours of approved continuing education which includes diabetes, educational principles, and behavioral strategies. CPR required. 3. Experience: At least 2 years exp. as a diabetes educator preferred. Commensurate with educational preparation demonstrates knowledge of and ability to apply age-specific principles of growth and development and life stages to meet each patient's needs for nursing care. 4. Interpersonal: Must have cooperative working skills, leadership skills, interpersonal skills, and the ability to work effectively and independently with a wide variety of patients, families and staff. Must be well organized and flexible. The ability to demonstrate a courteous, caring and understanding attitude toward patients, families, co-workers, visitors, physicians and volunteers is required. 5. Physical: Must have ability to: tolerate long periods of sitting, walking or standing; communicate verbally with individuals in person or on the telephone; read and write text; solve complex problems; respond quickly to department or hospital emergencies. Must also be able to: lift equipment and supplies weighing between 25 and 50 pounds; independently push and pull beds or equipment on wheels weighing up to 500 pounds to other areas of the hospital on occasional basis; bend and stoop frequently; read and interpret applicable diagnostic screens; hear and interpret, stethoscope sounds; use senses (vision, hearing, smell, touch) to recognize changes in patient's condition; hear codes; perform CPR in emergency situations. ESSENTIAL DUTIES - Coordinates and provides diabetes education for professional staff. - Coordinates and provides patient education - Utilizes KGH electronic medical record to access and document care - Coordinates activities of the Diabetes Program with the program Advisory Committee and Manager. - Participates in the budget process for the program each year. - Participates in an annual review of the effectiveness of the program, including planning for new efforts in the following year. - Serves as a member of the Diabetes Education Advisory Committee. - Promotes the Diabetes Education program within the Ketchikan General Hospital service area. - Patients and visitors are addressed in a friendly, helpful manner in accordance with KGH values. - Department safety and maintenance is ensured in a manner consistent with the hospital and department policies and procedures. - Performs other duties as directed in a cooperative and efficient manner. - Professionalism, clinical motivation and participation in continuing education are pursued to ensure employee effectiveness. - Achieves mutual goals as listed on the previous performance appraisal. - Personal dress, grooming and good housekeeping practices are maintained promoting a professional appearance. - Adheres to confidentiality policies and procedures without exception. - Adheres to the Mission and Values of KGH without exception.
