Description: Physical Therapist Per Diem- Weekend Coverage
Full time- looking for m-f coverage for inpatient and outpatient care. Occasional weekend.

OR

Per diem- Looking for weekend coverage- Saturdays and an occasional Sunday. Hours are flexible.

Coverage for a short term rehab unit in our skilled nursing facility.

- Develops and implements inpatient rehabilitative treatment programs that restore or significantly improve the patient's level of function, independence, and quality of life.
- Selects rehabilitative activities suited to patients' level of deficit, cognitive abilities, age and baseline abilities & needs.
- Establish treatment priorities in order to set short term and long term goals.
- Assesses potential barriers to learning, and patient learning preferences to ensure that the rehabilitative program is effective.
- Ensure that treatment goals are appropriate to patient's current needs.
- Regularly evaluates the patient's functional status and adjusts treatment plan accordingly.
- Maintains complete documentation of patients' evaluations, treatment, progress, and discharge status.

Requirements:
Bachelor's of Science in Physical Therapy or related field is required MS or dPT in Physical Therapy is preferred. NYS License in Physical Therapy is also required. The ability to effectively communicate both verbally and in writing is required. New Graduates are Welcome!
