Work as a half-time Registered Nurse in Health S.E.T.'s Health Promotion Clinics. Provide direct care to low-income senior citizens. Health S.E.T. is a nonprofit organization sponsored by Centura. Please see healthset.org for more information. •Assists in the preparation of long-range and short-range plans for clinics •Point person for working with building contacts to coordinate clinics within their buildings (responsible for setting up clinic dates and times and changing dates and times when there is a conflict) •Present at Resident Council meetings at each of the clinic buildings, once each year. During these presentations, educate residents on the services our organization offers and ask the residents for their input on unmet needs •Assists in the recruitment and orientation of clinic volunteers •Assists in preparing educational handouts for clients •Sets up equipment at clinic sites Clinic and Advocacy Responsibilities •Provides direct client assessment (i.e., blood pressure, pulse, weight and medication review) at each clinic and during home or provider visits. •Provides individual client education specific to client's health care needs •Works with staff to identify and meet clients' needs through interagency referrals and follow up •Assists clients in filling out applications for outside agency and financial assistance •Demonstrates timely follow up for client care •When possible, forms a working relationship with client's family and support system •Acts as an advocate for the client when working with the client's support system and agencies or organizations involved in the clients care •Documents client's data in paper files and computer records 1 to 5 years minimum experience, experience in community health a plus • Must be a Registered Nurse in good standing in the state of Colorado • Bilingual (Spanish/English) a plus Jennifer - Rec Caughlin JenniferCaughlin@Centura.Org Posting
