Provides holistic, quality and cost-effective nursing care for patients/families and/or others for a patient population within a unit, service or other primary area of practice with an awareness of the episode of care. Applies the nursing process within the context of the organization's Framework for Professional Practice. Functions as clinically competent. Delegates and supervises work appropriately. Reports to Unit Manager. Appropriately assesses and plans patient care by: accurately and reliably obtaining, assessing and interpreting data; identifying and documenting problems and expected outcomes based on a plan of care for complex, multi-varianced population of patients; utilizing scientific knowledge base and appropriate physiological/psychological/social/cultural/spiritual concepts; organizing care and interventions; and identifying appropriate resources in order to meet individual and family health goals. Using research-based recommendations, coordinates and implements patient-centered care according to a plan of care with variances by: assistance in establishing, contributing, and following plans of Health Care Team and educating patient/family about the plan of care; ensuring documentation and communication among caregivers; delegating and ensuring satisfactory completion of activities with Unlicensed Assistance Personnel and peers ( when in resource role) ; and educating patient/family to meet established health goals in order that all patients/families receive coordinated seamless care. Using reliable tools and measures, evaluates and documents patient/family responses, (including post-discharge/post transfer follow-up via measurement of nursing-sensitive outcomes), identifies complex variances from anticipated outcomes, and appropriately revises plan of care according to patient/family needs, monitoring outcomes across shifts for unit based stay and functioning as patient advocate as needed . Demonstrates competencies in Nursing practice (knowledge and behaviors), utilizing research behaviors, to meet age-specific patient needs. Provides/models quality patient/customer service by: demonstrating proficiency in basic computer skills such as operating clinical display and logging on, identification and navigation of clinical display, utilization of rosters and charts, troubleshooting and logging off, and email; demonstrating a willingness to accomodate all customers at all times; maintaining an awareness of patient needs and utilizing initiative to respond to those needs; utilizing tact, discretion, and compassion in responding to patient/family needs; maintaining confidentiality of all patient information; and functioning in a collaborative manner with other members of the team to ensure quality patient care. Contributes to care improvement by participating in: coaching, and acting as resource for peers; performance/quality improvement opportunities; demonstrating the cost effective utilization of resources (self/unit in resource role) and by recognizing the value of research findings as part of continuous improvement. Contributes to improvement of nursing practice by questioning current practices. Qualifications: RN CT License. RN Diploma, Associate Degree, BS in Nursing, or BS, BA( non- Nursing.) New graduates will require one year of experience to become competent, with an evaluation at 6 months to assess progress.
