Position scope: -Obtains thorough health and medical histories from patients. -Performs physical exams and assessments. -Reviews and orders inoculates and vaccinations for patients as required; reviews and updates immunizations per current guidelines. -Orders and interprets appropriate diagnostic tests including outside labs and imaging. -Identifies health and medical risk factors and takes an active role in preventive health screening and counseling. -Diagnoses health problems and treats accordingly. -Consults with physician colleague on patient specific case scenarios as appropriate. -Prescribes, monitors, and alters medication or initiates other treatments as appropriate. -Refers patients to medical specialist for consultant services when necessary for further evaluation and treatment outside of the provider's scope of practice or clinic facility capabilities. -Communicates with patients existing primary care physician on care performed through the clinic. -Collaborates with Medical Director to stay current with evidence-based protocols as foundation for care and per CareCorps®/Clinical Resources Group™ policy. -Provides ongoing preventive health education and counseling to patients. -Maintains medical records utilizing an electronic health record system (iSalus) in compliance with health care standards, documentation requirement, and accurate coding procedures. -Maintains process for systematic patient follow-up to determine effectiveness of treatment plan, reassessing and modifying plan as necessary. -Participates in health education programs and staff training sessions as requested to maintain and increase skills relevant to the patient population. -Provides patient coaching as needed. -Assists with the development and administration of worksite specific wellness programming as directed by Clinical Resources Group™ corporate office. -Attends and participates in client new employee orientation to educate and promote the services provided in the clinic. Position criteria: -Ability to provide quality service to all contacts, both internal and external. -Fully licensed medical provider in the respective state of practice without restrictions. -Graduate of fully accredited School of respective medical profession of the licensed provider. -Possess current, unrestricted Arizona license or ability to obtain. -Prescriptive authority in state of licensing (or eligible). -Certified in Basic Life Support. -Skills and knowledge to help patients make lifestyle improvements. -Working knowledge of electronic health records, clinic operational flow. -Minimum 3 years advanced practice experience strongly preferred. -Strong interpersonal skills; ability to work well with all levels of leaders and associates in clinical and corporate office environment. -Ability to pass a criminal background check and drug/alcohol test. -Ability to establish priorities and work independently. -Ability to work and collaborate in a team environment. -Ability to effectively mentor and provide guidance to other staff. -Good written and oral communication skills.
