Description
The Admissions Registered Nurse is responsible for providing information on hospice services to potential patients and families; admitting patient to the program with completion of all admission paperwork on a timely basis. Participates in the Interdisciplinary Group (IDG).

Principal Accountabilities
Communicates to Patient Care Manager/IDG and RN Case Manager the needs of the patient and family upon admission to assure comprehensive care.

Attends and presents patient admissions to IDG.

Makes initial assessment of the patient and family upon admission and reviews the hospice benefit.

Develops a written initial Plan of Care for each admitted patient in conjunction with the IDG, patient, family and attending physician.

Reports patient's admission date and findings to attending physician, and obtains verbal orders as needed.

Completes election of benefits forms accurately and timely.

Reviews services available with patient and family, including on-call services and volunteer services.

Coordinates admission visit with Patient Care Manager/Admissions Coordinator.

Communicates HHA/CNA needs to RN Case Manager or PCM.

Re-evaluates patients at time of recertification for continued eligibility.

Attends and presents patients IDG for continued eligibility.

Documents professionally, accurately, timely and according to VistaCare policy, procedures and guidelines.

Participates in on-call duties.

Adheres to VistaCare Standards of Conduct.

Has a duty to report any potential misconduct or violation of VistaCare Standards of Conduct or federal or state laws and regulations.

Has a responsibility to participate in staff training and education programs.

Performs other duties as assigned.
Requirements
Qualifications


The hours are 9-6 and we would prefer a BSN. Hospice exp strongly preferred.

One year of recent medical, surgical, oncology or home care experience.

Registered Nurse with current state license.

Complies with acceptable professional standards and practice.

Knowledgeable of pain and symptom control as related to terminally ill patients.

Demonstrates good communications, negotiation and public relations skills.

Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

Reliable transportation, current driver's license and current auto insurance.
