Job Details: Coord. Quality Review RN (FT)

Description: Serves as champion for physicians and staff and provides education to achieve concurrent compliance with core measures and other quality initiatives/ requirements.
Performs concurrent chart reviews for core measures and other quality initiatives. Collects, aggregates, analyzes, and reviews data for improvement opportunities. Assists with performance improvement processes as directed. Maintains strict confidentiality of all data and information.

1.0 Develop, coordinate, disseminate, and implement action plans to achieve compliance and improve core measures and quality initiatives.

1.1 Perform concurrent contact with physicians and document

orders, notes, etc. as directed by physician to achieve

compliance

1.2 Collaborate with physicians, nursing, case managers and other ancillary services to facilitate care management and discharge planning to achieve compliance in core measures and improve quality outcomes.

1.3 Collaborate with clinical educators, coders and other staff to provide education and serve as resource regarding core measures requirements and other quality initiatives.

1.4 Ensure appropriate and timely communication on core measure performance to achieve compliance.

1.5 Maintains expert knowledge of core measures, quality initiatives and researches for potential changes and implements proactive modifications/ education.

1.6 Report summaries of review results, feedback on potential quality issues and trends, peer review to Physician Performance Improvement Manager and Vice President Medical Affairs.

1.7 Perform inter-rater reliability (IRR) abstraction for core measures.

1.8 Perform abstraction of data as needed.

1.9 Assist with coordination of the quality and performance improvement processes.

1.10 Maintain strict confidentiality of all data and information.





50% 50% 2.0 Perform concurrent clinical chart reviews based on indicators, investigate cases referred, collect data and other information, etc. as required or requested. Conduct reviews, including at least the following, in a timely manner to meet review and reporting requirements.

2.1 CMS/JCAHO Core Measures review.

2.2 Assists with other quality initiatives.

2.3 Perform targeted/focused performance improvement review as requested.

2.4 Other reviews as assigned.

3.1 Maintain databases and review sheets as required.

3.2 Meet deadlines and timeframes for reviews and reporting.

3.3 Develop or review policies and procedures as required.



2% 2% 4.0 Comply with all hospital policies and procedures, safety requirements, and emergency preparedness requirements.

KNOWLEDGE, SKILLS, AND ABILITIES

As a champion of the core measures process and other quality initiatives, needs the ability to motivate, lead, delegate, organize and communicate. Ability to communicate effectively both verbally and in writing. Comprehend complex clinical data. Organize, prioritize and manage time efficiently to meet deadlines. Maintain and operate database and other software as required to perform duties. Maintain effective interpersonal relations. Ability to learn quickly and be flexible. Maintain confidentiality of all data, information, and activities. Ability to perform detailed, concentrated work with limited supervision. Must be able to demonstrate expectations regarding core measures and outcome data of CMS/ HCA and demonstrate understanding and compliance to all safety protocols and procedures

Skills/Requirements

Required:
-Bachelor's degree in nursing from an accredited university.

Preferred:
-Three years experience in clinical nursing. Experience in quality chart reviews, utilization management, discharge planning and/or case management
