The Diabetes Nurse Case Manager will be responsible for identifying best practice management strategies using clinical practice guidelines and AADE’s seven self-care behaviors for identified patient populations and striving to achieve cost effective utilization management for high volume and at risk for complex high cost disease/illness patients.

Qualifications
Graduation from an accredited school of professional nursing is required. Three years previous experience in medical/surgical/ICU nursing required. Recent experience as a registered nurse in a diabetes education setting is desirable. Adult education experience preferred. Demonstrated teaching experience and knowledge of adult education principles, ability to deliver and evaluate adult education programs. Current license from the Board of Nurse Examiners of the State of Texas to practice as a registered nurse is required. Current certification as a Diabetes Educator (CED) is preferred. Certified Diabetes Educator credential must be achieved within two years from date of hire. Must maintain valid CPR (BLS) certification.
