Nurse Practitioner I PD needed in Baldwin Park, California!
Qualifications:
-Bilingual (English/Spanish) Level II.
-Graduate of an accredited RN and NP Program Masters Degree (* effective 5/23/07 for those not previously grandfathered in)
-Current California RN License. Current California Nurse Practitioner Certificate
-Maintain current national certification from approved organizations (American Nurses Credentialing Center, American Academy of Nurse Practitioners, National Certification Corporation for Obstetric, Gynecologic and Neonatal Nursing Specialties, or the National Certification Board of Pediatric Practitioners)
-DEA Registration Number, if applicable.
-California Furnishing Number BCLS (ACLS, PALS If applicable)
-National Provider Identification Number (NPI) and be an approved Medicare provider.
-Minimum one (1) year experience unless specified by department.
-Consistently demonstrating the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our members, and to purchasers, contracted providers and vendors.

-Registered Nurse Practitioners must have UPIN or be eligible, which means Masters Degree in Nursing and National Certification (must be one of the following: The American Academy of Nurse Practitioners; American Credentialing Center; National Certification Corporation for Obstetric, Gynecologic and Neonatal Nursing Specialties; National Certification Board of Pediatric Nurse Practitioners and Nurses; Oncology Nurses Certification Corporation; Critical Care Certification Corporation).
Preferred Qualifications:
-State of the art knowledge & clinical management of patients with prevalent chronic conditions, i.e., HTN, DM, Lipidemia, CKD, HF, Depression.
-Ability to competently provide an integrative care delivery model.
-Three (3) out of the last five 95) yrs working directly in primary care as an autonomous RNP.
-Ability to work independently, self-starter, energetic &outcome driven.
-Direct experience in collecting, analyzing, trending & formally presenting data.
-Demonstrated competency in coding & documentation.
-Proficient with Kaiser mainframe, Health Connect, KPDS, POINT database & computer programs, word, excel, PowerPoint, access.
-Proficient problem solving skills, oral & written communication.
-Ability to work in a team environment,& assume full program accountability.
-Flexible, with a proven history of superior attendance, time management, Impeccable work ethics, nursing leadership.
-Previous care/case management experiences a strong asset.
Notes: Must pass the bilingual assessment test. Schedule varies. Occasional travel to meetings/activities off Baldwin Park Campus. Occasional weekends. Other departmental/organization tasks/duties as assigned.
