Nurse Practitioner opportunity in Falls Church, VA!
Schedule: Rotating on-call position to include holidays and weekends.
Purpose: To provide direct care to patients, under the supervision of a physician. This position is administratively supervised by the Clinical Coordinator or designee.
This position consistently supports compliance and Kaiser Permanente’s Code of Conduct by adhering to federal, state and local laws and regulations, accreditation and licenser requirements and Kaiser Permanente’s policies and procedures. In addition to defined technical requirements, accountable for consistently demonstrating service behaviors and principles defined by the Kaiser Permanente Service Quality Credo, the KP Mission as well as specific departmental/organizational initiatives. Also accountable for consistently demonstrating the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our members, and to purchasers, contracted providers and vendors.
Accountabilities:
1. Assesses clinical, emotional, and educational health needs of patients.
2. Assesses health needs of patients, including physical assessment.
3. Collaborates with physician’s plans of care and assures appropriate follow-up including telephone contact, home evaluations and hospital rounds.
4. Collaborates with the physician and other clinical staff members to maintain continuity of care.
5. Provides ongoing emotional support of the patient and his/her family both in formal groups and at each patient encounter.
6. Participates in research activities.
7. Facilitates referrals to appropriate community agencies including hospitals.
8. Participates in patient education activities.
9. Participates in orientation and education of nursing and support staff, and nursing and other students.
10. Collaborates with physicians in developing and periodically reviewing protocols relevant to department functioning.
11. Participates in planning for team functioning in delivering the total range of health services.
12. Participates in monitoring quality of care according to professional standards and established criteria.
13. Knows limits of practice and provides appropriate referrals.
14. Carries out clinical and administrative procedures that may be promulgated by the Health Plan.
15. Performs other related duties as directed.
Minimum Requirement - Relevant Years of Experience:
- One year of primary care experience is required. Two years of primary care experience is preferred- Proficiency in the use of applicable computer software.
Minimum Requirement - Education and/or Classes:
- Graduation from a NLN accredited NP program.
- Masters degree in Nursing required.
Minimum Requirement - License, Certification and/or Designation:
- ANCC certification required
- Qualified to obtain license to practice in the jurisdiction to which person is assigned.
