We provide 24-hour emergency and medical/surgical services at our Oakland and Berkeley campuses, and obstetric services in Berkeley. We currently have openings for four per diem, day shift, RN Case Managers
The RN Case Manager manages and coordinates resources necessary and appropriate for the achievement of optimal clinical and financial patient outcomes.
Requirements:
Bachelors degree with 3-5 years experience in case management, utilization management and/or discharge planning in an acute care setting OR RN with 5-7 years experience in case management, utilization management and/or discharge planning in an acute care setting
Valid CA RN license
Quality management or community health experience preferred
Knowledgeable in the clinical management of cardiovascular patients
Assertive and creative in problem-solving, system planning and management
Self-directed and able to work under pressure and constant change
