DEPARTMENT: Physical Therapy

SCHEDULE: On-call, 0 Hours per Week, Day Shift, Schedule to vary as required.Position Summary: Provides independent physical therapy management of patients under the directionof the Director of Rehabilitation Services or designee. Provides physical therapy services inspecialized patient care clinics, and assists in the development, implementation and review ofdepartmental patient care programs.

Education/License/Certification: Graduate of an accredited physical therapy curriculum with a Bachelors or Masters degree with certification in Physical Therapy or foreign-trained equivalent.

Licensed to practice physical therapy by the State of California. Must have a National ProviderIdentifier (NPI), or obtain an NPI, prior to employment start date.

Qualifications: Must have at least two years experience in physical therapy. Must be able to work in a Labor/Management Partnership environment.

Preferred Qualifications: BLS for healthcare providers from American Heart Association. Experiencedwith evaluation, assessment, appropriate goal setting, and planning treatment & interventions.dysfunction�s. Orthopedic skills & Sports medicine knowledge & skills very helpful. Able to adaptfor the neurological dysfunctions. Use evidence based practice. Willingness to teach variousaudiences (patients, members, staff) and students. Stay abreast of current practices &interventions.

Ability to move forward with ongoing quality assurance needs, pursue, participate, & developappropriate project pertinent to the department and Medical Center needs.Have computer knowledge & related basic skills.

Able to function competently in hospital arena of patient care as needed.Ability to communicate effectively and professionally, verbal and written. English communicationskills required. Ability to work in a fast paced setting and frequent interactions w/ others

Perform or pursue other duties as necessary & is able to work well in team environmentMust be able to demonstrate and maintain professional conduct and have strong customer serviceskills. Must have a good attendance and punctuality record and it is expected to be maintained.

Spanish language skills are helpfulDuties: Evaluates patients and interprets evaluation findings to determine the nature and extent ofdysfunction. Establishes treatment goals and plans treatment to achieve established goals. Initiatesdischarge planning for physical therapy. Implements the treatment plan through direct treatment ofthe patient, delegates a portion of the treatment plan to appropriate supportive personnel, andcoordinates the treatment program with other health care personnel as appropriate. Provides adviceand education to patient and family. Regularly reassesses clinical signs and symptoms to determineeffectiveness of treatment, progress of patients towards goals, and the need for modifying goals andtreatment. Discharges patient from physical therapy when the patient has received maximum benefitfrom physical therapy. Provides accurate, timely documentation of physical therapy patientmanagement. Participates in appropriate and approved educational and clinical research activities inphysical therapy. Participates in specialized patient care clinics outside the department. Assistsin the development, implementation, and review of departmental patient care programs. Performs otherduties as needed in the areas of staff education, departmental operations, and departmentalprojects. Demonstrates on-going progress toward meeting the criteria for professional practice.
