JOB SUMMARY: Provides health care services to children in the Early Education and Intervention Program, Head Start and schools associated within the Gila River Indian Community in consideration of the diverse physical, emotional, spiritual and socio-economic needs and beliefs of families and the community in accordance with all tribal, state, federal and licensing standards and guidelines MINIMUM QUALIFICATIONS: 1. Current State of Arizona RN license; American Heart Association BLS certification within 30 days of hire 2. Must have at least 2 years previous Public Health, Community Health, Pediatric or primary health care experience with school age children and/or school nursing program; current and valid State of Arizona driver’s license ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: 1. Provides age appropriate nursing care to the episodic, urgent, emergent, acute and/or chronically ill or injured student in a school setting 2. Performs assessment, triage, intervention, treatment, evaluation and referral as appropriate 3. Maintains and updates student health records through concise, accurate and timely documentation 4. Provides health services to children with special health care needs, including, but not limited to: chronic illnesses and disabilities to ensure integration of an individualized health care plan with educational plan 5. Assesses student’s health status, identifies health problems effecting educational progress and develops plan appropriate for school setting 6. Identifies, removes, minimizes and/or manages health issues of students to ensure students remain in school 7. Provides safe and effective direct health services and/or facilitates performance of special health care procedures required 8. Participates in student/family/teacher conferences; provides information and education regarding health issues to provide for understanding and facilitation of student’s educational needs 9. Provides mandated screening and immunization monitoring, health education and access to primary health care 10. Develops and maintains good working and professional relationships with school personnel and board members to facilitate open and cooperative communication 11. Promotes healthy and safe school environment by implementation of performing safety drills and environmental inspections 12. Serves as a direct link between students, families, schools, medical providers, community services agencies and other agencies 13. Participates in planning, instituting, evaluating and revising medical program plans; assists determine conditions, resources and policies for efficient delivery of health services 14. Maintains records and reports on patients, program activities and status associated with the program operations, quality assurance and outcome measures 15. Works closely and cooperatively with other health care providers and outside agencies including, but not limited to: Child Protective Services, RHBA, Tribal Social Services and OPD as needed to ensure timely communication of issues/concerns and coordination of care 16. Actively participates in the implementation and evaluation of annual health fairs, vision/hearing checks, immunization clinics and wellness screening programs 17. Implements and evaluates immunization program status to ensure compliance with all regulations and requirements 18. Maintains close contact with patient through home, school health office visits, phone and/or written to monitor progress and suitability of environment 19. Participates in departmental orientation, on the job training and quality assurance programs/initiatives 20.
