To find out more, go to www.chwcareers.org/glendale EOE POSITION REQUIREMENTS Glendale Memorial Hospital seeks an experienced Registered Nurse to promote appropriate palliative care for patients and their families who are living with advancing chronic illness. * Reports directly to the Director of Case Management/Social Services working in collaboration with Administrators, Medical Directors, Social Workers, Nurses and other staff. * Leads the process of system analysis, making recommendations as needed, functioning in a consulting role in the development of the palliative care program. * Acts as a resource for patients needing symptom/pain management as well as providing direct patient care. * Will meet program objectives as defined by the facility´s administration including education/training. * Takes initiative and works independently. * Conducts assessments, arranges and facilitates meetings/conferences as needed. * Performs duties in alignment with the mission, values and policies of the organization and in compliance with the performance improvement plan, Joint Commission and other licensing, accrediting and regulatory agencies. * Performs special assignments deemed appropriate to the growth and maintenance of the palliative care program.* Experience: - 5 years in acute palliative care setting or hospice. Education, training or consultation experience required.- Expertise in palliative care and advance care planningas well as pain and symptom management. - Strong clinical assessment background and skill.* Education: - Successful completion of a program of training, which conforms to standards, established by the California Board of Registered Nurses. - Current California RN license required.* Specialized Training/Skills: - Hospice, ELNEC, Oncology, Palliative Care certifications preferred. - Current BLS certificate. - Excellence in oral/written communication.
