Knowledge and Skills: Required for completely satisfactory performance in this job is a fundamental knowledge of pediatric assessment and treatment. Also required is knowledge of related OSHA, CLIA and JCAHO guidelines and requirements, general office tasks, confidentiality and filing system. Also required in this job is the ability to use thermometer, scales, sphygmomanometer/stethoscope, scalp vein needles, hearing/vision screening equipment, pulse oximetry, typanometer, basic laboratory, i.e., microscope, centrifuge, hematology analyzer, and specimen collection. Proficiency with treatments such as nebulizer, ear irrigation, and dressing changes. Familiarity with all business office machines, i.e., computer, multi-line telephone, copier and fax machines. Formal Education, License and Experience: The formal education normally associated with completely satisfactory performance in this job is a high school graduate, GED or equivalent. Graduation from an accredited school of Licensed Vocational Nursing. Current license or permit to practice vocational nursing in the State of Texas. Current CPR. No minimum years of experience are required.

Under the direction of the Director of Nursing and in collaboration as a member of the multidisciplinary team, performs assessments, plans and provides medical care and performs nursing procedures. The LPN operates in accordance with the competency guidelines and full scope of practice within the Rules and Regulations of Registered Nurses outlined by the Tennessee Board of Nursing and according to agency operating policies, protocols, and standards. Requirements Must be a graduate of an accredited school of practical nursing and be licensed by the Tennessee Board of Nursing to administer care, working under direction of a licensed physician. Prefer two years of experience in nursing and experience in a residential or school setting serving children.

To learn more about us and the Transitional Care Centers, visit our website at www.intercedehealth.com Position Description:• Responsible for patients assigned to the Conroe and 1960 Clinic sites, including: o Scheduling of appointments, routine and urgent o Management of all ordered labs, tests, and referrals, including scheduling, authorizations, compliance, and communication of results to ordering provider o Coordination of all ordered care services including communication with patients, physicians, health plans, pharmacies, and vendors o Maintenance and organization of all medical records and charts for assigned patient populations o Coordination of medications, prescriptions, and refill requests for all assigned patients o Routine communication with patients including missed visit calls • Provision of all patient services at Conroe and 1960 Clinic sites during office hours, including: o Patient check-in o Rooming patients o Vital signs and medication log o Assisting physician, nurse practitioner with treatments and services o Administration of medications, injections, vaccines, or tests as ordered by provider o Education support for patients and family members o Patient check-out • Participation in educational opportunities including staff meetings, group presentations, and health plan orientations Requirements Position requirements/pre-requisites:• Current, valid Texas License as a Licensed Vocational Nurse • Current, valid CPR certification • Minimum 3 years work as an LVN in an outpatient setting • Bilingual English/Spanish is preferred • Reliable transportation for commute to different office sites • Excellent people skills as it is essential to work well with team and with patients and their families • Experience working with Medicare patients and families • Experience working with Texas HealthSpring, Texan Plus, Amerigroup, and other Medicare managed care programs a strong plus • Ability to use MS Office applications and prior experience with an electronic medical records system
