The Case Management/UR Nurse will be responsible for providing concurrent review for ongoing medical necessity using established SI/IS criteria. Evaluates, plans, coordinates, reviews, and revises patient discharge plan based on patient need in collaboration with the physician and the multidisciplinary team. Coordinates care delivery processes and promptly intervenes in instances of delayed services or inappropriate utilization of resources. Must be clinically current and able to demonstrate knowledge of commonly used utilization criteria. Ability and flexibility to work in all areas of the hospital. Must have a current RN license. At least three years recent experience as RN CM in Acute Care Hospital is preferred. Experience in NICU/Surgery Case Management is preferred.
