JOB SUMMARY:Within the context of a client and family centered care model and in accordance with the College of Registered Nurses of British Columbia (CRNBC) or the College of Registered Psychiatric Nurses of British Columbia (CRPNBC) standards of professional practice, Code of Ethics, and the established vision and values of the organization, works as part of a multidisciplinary team to provide outreach services to individuals with serious mental health and/or addiction issues in the community. Assess high risk situations and provide crisis intervention. Provide counseling; assist client with reducing repeated contact with acute and community healthcare resources and with utilizing services/programs/resources. Conduct mental and physical status assessments on individuals experiencing a serious mental health and/or addictions issues. Assess clients physiological, psychological, sociocultural and spiritual needs. Determine priority of care requirements based on client care needs, availability of resources and best practices; collaborates with interdisciplinary team. Develop implement and adjustment care plans. Participate in case conferences/meetings with members of the multidisciplinary team. Assist clients with attendance to needed services such as medical appointments.QUALIFICATIONS:Diploma in Nursing or Psychiatric Nursing. Two (2) years recent related experience working with individuals with mental health and addictions illness, or an equivalent combination of education, training and experience. Current practicing registration with the College of Registered Nurses of British Columbia (CRNBC) or the College of Registered Psychiatric Nurses of British Columbia (CRPNBC).
