Ensure coordination of care across the continuum for Breast Center patients. Facilitates diagnostic, post-operative and/or treatment follow-up care. Collaborates with members of the health care team to facilitate the transition and coordination of follow-up after clinic visits, and/or follow-up by consulting services. Functions as the educational resource to staff nurses and other health care providers in the disease unit and/or quality initiatives.Requirements:Bachelors degree in Nursing required. The Nurse Coordinator for Oncology Multispecialty Care Centers (OMCC) is an integral member of an assigned Disease Team.

The Nurse Coordinator for Oncology Multispecialty Care Centers (OMCC) is an integral member of an assigned Disease Team. The OMCC Nurse Coordinator focuses on: 1) navigating the patient and family across care settings according to the established patient care plan2) ensuring that the patient/family have appropriate information, understand and have the capacity to adhere to the medical plan3) bridging between the various consultants, services, points of care encountered by patient/family. The primary roles of this provider are serving as a consistent primary contact for new patients and their families, partnering with the Intake Assistant to expedite entry of new patients into the system and collaborating with Disease Team members to ensure coordination of care across the continuum for the designated patient population. The OMCC Nurse Coordinator synchronizes patient/family care across care settings beginning with the initial referral and consultative visit and ending with handoff at a designated point determined by the specific disease team (will vary according to patient population and disease team preferences). The OMCC Nurse Coordinator interfaces with point of care staff in accordance with the plan for patient care, educates patients and families based on determination of learning needs, and communicates plan detail to patient/family, physicians, mid-level providers, others involved in the patients care. Bachelors Degree in Nursing highly preferred; Masters Degree in Health-related field preferred. A minimum of three (3) years experience in adult inpatient services, ambulatory oncology service or operating room. Oncology experience preferred. Connecticut RN license; Oncology Nursing Society certification (OCN, AOCN) desirable. Knowledge of physiology and disease involving the specialty disease team and familiarity with tumor staging. Excellent communication skills. High level of organization. Experience caring for hematology oncology and stem cell patient populations.

As a core member of the care delivery team, the Clinical Nurse practices as a primary and/or associate nurse and collaborates with the team to facilitate the coordination of patient care at the unit level. Through utilization of the nursing process, the clinical nurse diagnoses and treats patient and family responses to actual or potential health problems. The Clinical Nurse is accountable to the patient, family, peers, and clinical manager. Graduation from an accredited School of Nursing. Licensed and a currently registered nurse in the State of Connecticut; minimum of 1 year current acute care RN experience required, ICU experience preferred. Clinical Nurse is the basic level for new graduates and all registered nurses who do not choose to participate in the Recognition and Clinical Advancement Program (RCAP).

Oncology Assistant Head Nurse

The staff of the Norma F. Pfriem Cancer Center has the unique experience to work with both inpatients and outpatients. The unit is composed of 19 inpatient beds and a 4-station outpatient service. The outpatient service is flexible in both volume and hours of operation, offering service 7 days per week, 24 hours per day.Oncology patients receive care through all stages of the disease process from diagnosis, through the end stage of life. The Women´s Care Center is located on the fifth floor of Bridgeport Hospital and is comprised of 36 beds with a 24 bassinet nursery. This unit serves women of childbearing age following a vaginal delivery or cesarean birth and provides well newborn care. Additionally, there are Special Care Maternity beds allocated for high risk ante partum and postpartum women. Gynecological surgical patients are also located on this unit for postoperative recovery. Ongoing family educational programs are provided during the ante partum and postpartum periods for both inpatients and outpatients. Assistant Head Nurse Emergency Department

Bridgeport Hospitals Emergency Department is a Level 1-trauma center, seeing approximately 62,000 patients per year. The unit is open 24 hours a day, 7 days a week to all patients regardless of ability to pay. Specialty areas include an adjacent separately staffed Urgent Care Center, a Pediatric ED open daily from 4pm to midnight, and a Chest Pain Center. The Emergency Department also has a Psychiatric Crisis Program, staffed with psychiatric nurses daily from 9:00am to 9:30pm. Total bed size is 36, including 3 behavioral modification rooms, 2 trauma/code rooms, 10 sub-acute bays, 8 holding bays and 5 observation beds. Sixteen of the beds are capable of providing telemetry. The patient nurse ratio ranges from 1:1 to 1:5 based on patient acuity. Most commonly treated illnesses include asthma, abdominal pain, chest pain, lacerations and fevers. Additionally, there are ongoing clinical trials, chiefly related to the treatment of stroke patients. The average daily census is 170 patients; approximately 18% require admission. The Cardiac Telemetry Unit is a 30-bed unit, which operates on a 24-hour basis located on Richardson 10. Cardiac telemetry monitoring is available for 30 beds.Medical Diagnoses of the adult patients cared for on Richardson 10 are: R/O Myocardial Infarction, Myocardial Infarction, Unstable Angina, Congestive Heart Failure, Syncope, Ventricular Tachycardia or Ventricular Fibrillation, Atrial Fibrillation or Supra-Ventricular Tachycardia, Implantable Cardioverter Defibrillator (new or malfunctioning), Pacemaker (new or malfunctioning), Drug toxicity or overdose. Patients who present with a medical diagnosis that is not cardiac in nature but have a history of arrhythmia or coronary artery disease may be admitted to this unit.Treatment modalities for patients admitted to this unit include Electrophysiology Studies, Electrical Cardioversion, initiation and adjustment of therapeutic pharmacologic regimens so that cardiac function is maximized, arrhythmias controlled and myocardial ischemic episodes minimized, and implementation of a cardiac rehab program that will assist the patient/family with understanding of the disease process and lifestyle changes associated with coronary artery disease and congestive heart failure.Works collaboratively with the NM displaying leadership in management, clinical practice, performance improvement and staff development in accordance with the overall philosophy and goals of Patient Care Services.
