MedAssurant, Inc. is seeking Nurse Practitioners in the Michigan area, with the direct responsibility of assessing and coordinating healthcare for patients with diabetes, heart failure, asthma and other chronic conditions in their disease management clinics. These clinics are associated with MedAssurant“s Michigan In-Market program. The In-Market program is directly correlated to CCS Advantages Disease Management Communications Center. The ideal candidate will possess an RN license in the state of Michigan and a certification as a Nurse Practitioner, MSN preferred.Responsibilities include:-Fulfill the clinical communication role between the Company, healthcare providers, and members/patients of clients of the Company; -Identify the need and implement chronic care management interventions as guided by pre-established protocols and procedures, which includes conducting face-to-face clinical assessments of patients, educating and coaching patients in the area of Diabetes including Asthma, COPD, CAD, Heart Failure and/or Hypertension and making necessary referrals to appropriate healthcare providers;-Critically evaluate each patients clinical case through reviews of each patients treatment plan and medical records/information; -Support the physician/patient relationship through ensuring physicians or providers are: sent appropriate documentation or reports following patient encounters, provided medical care in accordance with national guidelines, and educated on the intent of the program and individual patient goals;-Care Managers must be able to travel among facilities within a specified region of Michigan; minimal travel to other locations outside of specified region. Travel requirement up to 80% in specified region of Michigan.Qualified candidates should have 5+ years experience in a clinical environment and possess extensive experience in chronic care including diabetes, asthma, COPD, CAD and cardiac disease. Candidates must also be knowledgeable of the geographic area, transportation trends and demographics of Michigan. In addition, qualified candidates must be creative, resourceful, team-player, dedicated and have the ability to set priorities and work on multiple tasks simultaneously.
