Title: CCA/PCW Job Status: Casual Location: Halifax - NS Post Date: Jun-01-09 Description: The Continuing Care Assistant/ Personal Care Worker is responsible to assist Comcare clients with the tasks of daily living related to personal care and to household management as assigned by the Client Service Coordinator/Supervisor. Primary goals are promotion of client dignity, independence, comfort, mobility, personal appearance and safety. The CCA/PCW is a member of the community health care team, participates in the establishment of the service plan and reports changes in the client´s condition. CCAs/PSWs adapt to varying degrees of supervision and to the client´s requirements and wishes while working within the established service plan. They verify the service they have provided in a written record and submit client reports according to procedure established in the local office.
