Patient/Member Educator, RN
Qualifications: Bachelor´s degree or equivalent experience (usually 4 years) required; BSNpreferred. Previous teaching experience required or previous recent experience in education inclinical service area (usually two to five years.) Adult learning and behavior change knowledgerequired. Demonstrated ability to utilize interpersonal, written and verbal communication skills andgroup facilitator/process skills. Computer literacy required. Current California RN licenserequired. Must have current BLS, CPR certification. CDE preferred or CDE eligible within 12 monthsof hire.Preferred Qualifications: Master´s Degree preferred. Assessing/teaching patients withdiabetes/CAD/CVD, adult learning theory, and PC applications (Microsoft office/PowerPoint/Excel).Classroom and 1;1 instruction.Duties: Conducts educational needs assessments to design and develop programs to meet regulatoryrequirements and organizational, departmental needs. Coordinates implements and evaluates programs.Communicates and markets education programs to a broad target audience using appropriate methods,and based on identified/assessed needs. Makes assessment of physiological and /or functional statususing protocols, initiates appropriate interventions. Develops individualized member/patient/familyeducation plan focusing on self management, delivers member/patient/family education (individual orgroup) specific to disease state. Provides direct group or one on one health education services tomembers and the public. Reviews and recommends high quality, culturally appropriate written andaudio visual health content. Consults with physicians and other health care providers re: member/patient specific clinical and health related issues and education needs. Participates inmulti disciplinary care teams and committees. Collects data; plans and prepares reports forperformance improvement and/or quality assurance audits. Makes recommendations for cost/qualitystrategies, works within budget. Serves as expert clinical resource to staff and departments indefining and meeting educational, clinical, quality and cost outcomes and performance measures.Participates in establishing department policies and protocols that support regulatory standards (JOINT commission, NCQA, MediCal). Consistently supports compliance and the Principles ofResponsibility (Kaiser Permanente´s Code of Conduct) by maintaining the privacy and confidentialityof information, protecting the assets of the organization, acting with ethics and integrity, reporting non compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanente´s policies andprocedures.Notes: Excellent customer service skills required, travel to off-site locations. The work schedulemay vary depending on program needs: including one or more evenings per week and/or weekends. Musthave dependable transportation. Be able to lift and move at least 50 pounds with correct bodymechanics. Be able to work in collaborative, multi-disciplinary teams, and have excellent customerservice skills. Must be able to work in a Labor/Management Partnership environmnet.
