The Clinical Case Management Coordinator is responsible for the screening, assessing, planning, organizing and the Coordination of all aspects of continuity and transitions of care for patients in the Emergency Department.If you have a burning desire to use your clinical nursing experience to facilitate the progression of care, we have redefined the role to satisfy your need to be an articulate, vital contributor to successful patient outcomes. You´ll partner with Masters-prepared Clinical Social Workers, interact closely with physicians, examine the verity of the illness and the intensity of services needed, and be inspired by a supportive learning environment. Above all, you´ll be respected as a knowledgeable clinician who takes this role seriously. Current California RN license, or Current California Social Work License, MSN or MSW required, Case Management Certification preferred. 3 years of acute care case management experience with demonstrated clinical expertise in patient care management, problem solving, critical thinking, research methodology, health care delivery systems, and financial/reimbursement issues.
