Cedars-Sinai Medical Delivery Network, an affiliate of Cedars-Sinai Medical Center, one of the nation’s premier healthcare institutions, has an exciting opportunity for an ambulatory case manager. Due to growth and program development, you have the opportunity to become part of a dynamic team responsible for coordinating care for patients in an outpatient setting. These functions are performed on-site or by telephone, under the supervision of the Utilization/Case Management Department and Utilization Committee.You will maintain the continuity of care for high-risk patients in an outpatient setting, ensuring that services are provided within parameters of established contracts and the patient’s health plan benefits. You will help develop, implement, and evaluate processes when identifying high-risk patients within our medical group and IPA. You will also coordinate provisions for ambulatory care by referring patients to such resources as home health, social services, behavioral health, health education and community resources.To qualify, you will have current California State RN or LVN licensure, 3-5 years of acute care nursing experience and 3-5 years of ambulatory case management, utilization review, and/or discharge planning experience. Disease management and/or patient education experience preferred. The position requires knowledge of medical/nursing standards of care as well as the ability to effectively collaborate with physicians, patients/families, and ancillary staff.
