Practice Description: The Austin Medical Center Mayo Health System is seeking Certified Nurse Practitioner interested in as needed (PRN) hours in the Urgent Care Clinic.The Austin Medical Center is an integrated medical center with a staff of 70 physicians and a 80 bed hospital. The Austin Medical Center, affiliated with the Mayo Health System, is physician led and recently completed a major expansion and remodeling project.Qualifications required: CNP: Master’s degree in nursing with certification as a Certified Nurse Practitioner required. Current MN license as a Registered Nurse required. Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) required. Ability to satisfactorily complete credentialing and privileging requirements.

This position will work closely with and support patients, physicians, home care and hospice staff and other referral sources and will need to be able to establish and maintain strong working relationships with these groups.This position requires a current Minnesota LPN license along with two years recent experience (within the last five years) in either home care or hospice with intake, staffing, patient care/planning involvement, or related experience. Knowledge and understanding of home care and hospice services and criteria is preferred. This position also requires proven customer service skills, excellent computer and typing skills and strong analysis and problem solving skills. UMPhysicians seeks a board-eligible/board certified Nurse Practitioner to join our practice at our MN Heart Clinic locations in Edina and Burnsville. The NP provides and coordinates patient care for adults with selected cardiovascular diseases throughout the course of evaluation and treatment, including hospital admission, inpatient care, discharge planning, transitional care, clinic follow up, providing information to referring physicians and institutions and patient/family education.- A qualified candidate will be a graduate of an accredited school of nursing and formal Adult Practitioner training program.
