Our Columbus Ohio office is actively seeking RNs with strong disease management experience!! Our Health Coaches provide telephonic outreach to disease management plan members. In this position, the Health Coach will conduct assessments and develop treatment plans and interventions; help the client set both short and long term goals; and monitor progress towards goal attainment. The coach will provide positive reinforcement, advice, and support. The position is 32 hours per week and eligible for benefits. Hours vary from 8am-8pm. ESSENTIAL FUNCTIONSActivities include assessment, problem identification, goal setting, and implementation of interventions Communicate with member regarding needs and develop mutually agreeable care plansEducate the participants about their disease state as well as other health issues relating to their plan of care Collect clinical data and enter in the APS customized system for tracking and metrics reporting purposes Monitoring for quality of care and outcome improvement in accordance with clinical indicatorsEDUCATION REQUIREMENTSGraduation from accredited nursing program. BSN preferred.Current unrestricted Ohio RN license required.EXPERIENCEMinimum of 3 years experience in chronic/disease care management and/or primary care case managementStrong background in disease management. Experience managing members with chronic illness, behavior modification, goal setting and evaluationSKILLS Exceptional verbal and written communication skills Ability to multitask in a call center environment is required. Position PurposeThe Transitions Coordinator will work closely with the Director of High Street Hospitalists; Director of High Street Primary Care Clinic; High Street Clinic nursing, social work, and office staff; P/SL nursing and case management staff; hospital and clinic physicians (including residents); and patients, caregivers, and families to help create, implement, and oversee a new Transitions of Care project and system at Presbyterian/St. Luke´s hospital and High Street Primary Care Center.Essential Duties/Responsibilities1.Coordination of care by multiple stakeholders in the process of Transitions of Care including: clinic PCP, hospitalist, High St. clinic staff, residents, hospital nursing, hospital case management, hospital and outpatient pharmacies, and care facilities. Duties may include but are not limited to:Assessing discharge needs at the time of admission Interfacing with hospital case management throughout hospital stayCoordinating efforts with the High St. social worker and behavioral health expert to optimize ongoing care and the use of community resourcesActing as liaison between hospital physicians and clinic staff for purpose of gathering and disseminating additional clinical data throughout hospital stayPerforming telephone follow-up (possibly aided by a clinical pharmacist) within three days of discharge from the hospitalEnsuring compliance with Transitions of Care algorithms and checklists (as developed by the High Street Transitions of Care Workgroup) prior to all dischargesDeveloping relationships with High Street inpatients and their caregivers/families to provide tools to actively participate in a safe transition of care from the inpatient to outpatient settingEnsuring documentation and communication of future outpatient plan of careTracking patients to their first post-transition outpatient visit to provide continuity of care and gather information that can be used to continuously improve the system2.Work with the High Street Transitions of Care Workgroup team in ongoing development of a system for transitions of care for High Street patients3.Day-to-day oversight of the implementation of the Transitions of Care system4.Aid in data gathering and record keeping with goal of showing feasibility, quality, and savings of such a system with the goals of expanding implementation to a broader population in the hospital and scholarly publication.Assisting in new and ongoing research on the topic of Transitions of Care in the medical setting.Acting as a champion for safe, high-quality Transitions of Care at P/SLRequired Qualifications-Minimum 3 years experience in hospital case management-Minimum 3 years experience in clinical Nursing, preferably a hospital based RN -Aptitude with basic word processing and spreadsheet software programs-Outstanding interpersonal skills with an ability to interact with stakeholders with a broad range of educational and communication skills, interests, and levels of investment-Outstanding organizational skillsPreferred Qualifications-Fluency in Spanish helpful but not requiredPersonal QualificationsThe successful candidate will be an individual with outstanding interpersonal skills who is energetic, conscientious, highly motivated, and creative. The Director of Case Management is responsible for the overall coordination and integration of the case management and social work programs. The Director provides administrative oversight of assigned areas and programs to meet the hospital tripartite mission of patient care, research and education. Frequent interaction with hospital professional staff and medical staff is needed to assure the Case Management and Social Work Programs are meeting the needs of patients. The position will be responsible for redesigning, administrating and maintaining the Case Management and Social Work care delivery models to optimize care coordination of patients across the continuum. In addition, this position will develop, analyze and maintain appropriate care coordination data bases for measuring outcomes and achieving defined goals.This position assists the hospital to identify and mitigate delays in service, gaps in care coordination, enhance communication between departments and providers, identify and promote appropriate use of hospital services and is an integral part of the interprofessional care delivery model. The incumbent is challenged to administrate an effective, efficient care coordination program that assists the hospital meet its quality and financial goals. Bachelors degree in NursingMaster´s Degree in Nursing or related field required (i.e. MBA or MHA is acceptable)Three to Five years of nursing experience related to case management required.Three to five years of leadership experience preferred.
