Establishes a patient specific plan of care in conjunction with the primary physician, clinical case manager and other appropriate health care providers. Provides direct patient care as outlined in the patient specific plan of treatment and physician’s orders. Provides care and administers medications according to established policies, procedures and protocols. Provides all components of care in a professional, goal oriented, patient specific manner, functioning within the Nurse Practice Act. Counsels and instructs the patient and family or care giver in meeting patient care needs, including safety. Documents all components of care rendered according to established policies and procedures. Provides both verbal and written communications to the patient’s physician. Communicates any changes in patient’s condition to the clinical case manager and the patient’s physician. Complies with established quality assurance guidelines. Complies with established personnel policies and procedures. Participates in in-service training programs. Obtains CEU credits as needed to maintain current licensure.
