TITLE (REQ ID): HOME HEALTH RN PD B (81094) DEPARTMENT: SKILLED NURSING CARE-HHA EMPLOYMENT TYPE: PER DIEM SHIFT: DAY HOURS PER PAY PERIOD: 0 SHIFT HOURS: 8HOUR FACILITY: DOMINICAN HOSPITAL SANTA CRUZ LOCATION: SANTA CRUZ, CA POSITION REQUIREMENTS RN with home care experience for per diem availability. Must have a current CA RN license and CPR. Must possess vehicle to visit patients homes and liability insurance. Must possess intermediate computer skills for use of laptop for charting and knowledge of OASIS and Title 22. Under the direction of the Clinical Manager and Case Manager, perform skilled nursing visits for wound care, IV therapy, and much more. Must follow or develop plan of care initiated by physician referral. Excellent time management and organizational skills required.

SALARY: $6427 - $7846; Approximate Monthly Equivalent; Paid Biweekly.5% additional pay for evening shift work; 7 ½ % additional pay for night shift work.APPLICANTS MUST BE WILLING TO WORK ANY SHIFT OF A 24-HOUR PERIOD AND ANY 5 OF 7 DAYS.QUALIFICATIONS: Licensure: Current license to practice as a Registered Nurse in California AND current CPR (cardiopulmonary resuscitation) certification. Other permanent and temporary appointments will be made as needed.ESSENTIAL FUNCTIONS: Under general direction; Assesses patients during initial assessment visit for current medical and psychosocial status, appropriateness of care and criteria for acceptance. Establishes a patient specific plan of care in conjunction with the primary physician, clinical case manager and other appropriate health care providers. Provides direct patient care as outlined in the patient specific plan of treatment and physician’s orders. Provides care and administers medications according to established policies, procedures and protocols. Provides all components of care in a professional, goal oriented, patient specific manner, functioning within the Nurse Practice Act. Counsels and instructs the patient and family or care giver in meeting patient care needs, including safety. Documents all components of care rendered according to established policies and procedures. Provides both verbal and written communications to the patient’s physician. Communicates any changes in patient’s condition to the clinical case manager and the patient’s physician. Complies with established quality assurance guidelines. Complies with established personnel policies and procedures. Participates in in-service training programs. Obtains CEU credits as needed to maintain current licensure. Assists in orienting other nursing personnel.Other Functions: Performs other job related duties as assigned.
