Responsibilities: Minimum Education: Nursing Degree from an Accredited Nursing School. Minimum Experience: 5 years recent clinical experience in acute, home health, hospice or managed care. Preferred Experience: 3-5 years experience in Case Management, Utilization Management or Discharge Planning, in acute care. Required Certifications/Licensure: Current Registered Nurse License in the State of Arizona. Certified Case Manager, CCM. Special Skills: The optical knowledge base of nursing process, clinical pathways, continuum of care and case management methods and standards. Excellent communication and documentation skills.
