We aim to provide our patients with the best available health care. Each patient is assigned a primary care provider, who is part of a working team that includes both attending and resident physicians, physician assistants, nurse practitioners, nurse mid-wives, nurses, medical assistants and skilled support staff. If a patient needs specialist care, a BMSNHC staff specialist may be available; if not, the primary care physician will make a referral.We have Spanish-speaking staff on hand to assist you with your medical needs. Interpreter Services are also available upon request for Vietnamese, Russian, French and American Sign Language.The Baystate Mason Square Neighborhood Health Center offers a wide variety of primary care and specialty care services for people of all ages, from birth to adulthood. Programs include: HIV Program - HIV counseling, management, confidential testing, medical care and case management, HIV/AIDS Counseling and Testing - for high risk populations (intravenous drug users and their partners, as well as partners of HIV positive individuals). Hepatitis Assessment, screening and testing during regular hours. Position:The Nurse Practitioner performs a variety of patient care activities including physical examinations, patient assessment and treatment, disease prevention and health education. Collects and assesses patient data through history gathering interviews and focused physical examinations to formulate appropriate patient centered plan of care. Implements a plan of care utilizing appropriate diagnostic studies and treatments, conducts follow-up examinations to monitor and evaluate the plan of care. Provides patient/family education and/or counseling and evaluates the outcomes of care using the clinical skills and judgments inherent in physician assistant education and practice.The incumbent will have knowledge of physical assessment in primary care and patient care. Excellent communication and patient teaching skills required. The ideal candidate will possess qualities that include value and respect for teamwork, professionalism, excellent interpersonal and communication skills, and a commitment to excellent patient care. Qualifications:Must be able to independently practice and manage complex patient population in a fast-paced environment. The incumbent must be knowledgeable about current evidenced based practice, chemotherapy toxicities, quality improvement, research, and able to partner with multiple disciplines. Must be graduate of an approved Nurse Practitioner program. MA license (or eligible) required. Oncology experience and certification is preferred.
