Baystate Health was also named as one of the Top 100 Most Highly Integrated Healthcare Networks in the US by Verispan for the ninth consecutive year.Position:Reporting to the Director of Adult Hospital Care and Operations, the Director is accountable for the effective programmatic redesign and development of a system-wide care management model across Baystate Health, and the administration, implementation and operation of the Baystate Medical Center Care Management Program. The Director provides effective leadership, programmatic development and oversight of the comprehensive care management program including case management and social work to achieve efficient and effective patient care and utilization management while ensuring that operations are in compliance with the required standards and consistent with the objectives and goals identified. In addition the Director is responsible to design and develop roles and responsibilities, create and maintain effective collaborative partnerships with leaders within and across Baystate Health in order to lead and execute effective care management plans and to promote efficient hospital care with regard to care coordination and discharge planning.Qualifications:RN with Bachelor degree or MSW. In addition a Masters degree or matriculation in nursing, business, MSW/LICSW program or other appropriate field is required. Certification in Case Management or related field required. Certification in Case Management Administration is required to have or must be obtained within 18 months of assuming position. Qualified Candidates should have exceptional leadership experience in fast past organization, demonstrated ability to develop partnerships and collaborative relationships with and across disciplines and organization and effective project management, quality improvement and utilization management skills. Strong computer skills (MS Office, Word, Excel) and experience with data management necessary.Demonstrated team building including integration of social work and case management, and leadership skills in complex organization is strongly preferred. Five years clinical experience with a minimum of 3 years of care management leadership required.
