We are currently seeking a Director of Care Coordination, RN.As Director of Care Coordination at Community Regional Medical Center, you will assume responsibility for all aspects of patient flow and inpatient care coordination, proactively taking steps to ensure the highest quality of patient care in a timely manner. Managing multiple major areas/processes, reviewing and revising functions for maximum efficiency and developing budgets and plans to achieve goals will be expected, as well as establishing positive relationships with Care Coordinators and Housekeeping Leaders, and quickly adapting to changing circumstances. This position reports to the Director of Operational Excellence. The individual we seek is a Registered Nurse holding a BSN degree (MBA or MHA preferred) and current OH RN licensure. Your background must include 3-5 years successful management/team leadership experience and a proven track record of developing positive, collaborative relationships with physicians, peers, employees at all levels and the community. A dynamic, take-action professional, you must have strong visioning, planning, development and implementation skills; sound judgment; and the ability to effectively resolve problems and make good decisions.
