The Nurse Case Manager is directly responsible to the Program Coordinator for case management functions and other outpatient program responsibilities. Incumbents will carry an individual case load of veterans including intensive case management and community case management.Duties include assessment and evaluation of the clients stated needs, latent abilities and social networks; screens and identifies clients appropriate for case management; provides health assessments and facilitates access to health care based on individual clients need; administers meds and provides consultative support to peers for medication management; provides crisis intervention; collaborates with families and other caregiver on an on-going basis to ensure the highest standards of care are practiced and the clients needs are met.Incumbent must be willing to drive a government vehicle for home visits, community outings routinely and be able to physically move patients belongings if necessary.

This is a temporary position, not to exceed 2.5 years. The Registered Nurse/Post-Traumatic Stress Disorder/Primary Care Case Manager functions as a case manager, consultant and educator while providing collaborative care to community based outpatient clinics (CBOC) patients. This nurse will perform assessments and provided collaborative for the study entitled Telemedicine Outreach for Post Traumatic Stress in CBOCs (TOP). The care provided by this professional nurse is performed via telephone contact with patients referred by CBOCs in Longview, Texas, Texarkana, Arkansas, and Monroe, Louisiana based on screening results and/or request from their primary care provider.
