TITLE (REQ ID): CASE MANAGER - RN (78673) DEPARTMENT: Case Manager EMPLOYMENT TYPE: FULL TIME SHIFT: DAY (VARIED) HOURS PER PAY PERIOD: 80 SHIFT HOURS: 8HOUR FACILITY: DOMINICAN HOSPITAL SANTA CRUZ LOCATION: SANTA CRUZ, CA POSITION REQUIREMENTS Under the direction of the Case Management Director and Chief Medical Officer, manages the continuum of patient care and appropriate use of resources; this includes evaluation, monitoring and modifying the treatment plan to achieve optimal patient outcomes. Performing admission and continued stay assessments with Inter Qual criteria and facilitating the transition to post-hospital care services (discharge planning) and participating in Continuous Documentation Improvement (CDIP) initiatives. MINIMUM REQUIREMENTSEducational Requirements: - Graduate of a state accredited school of nursing, B.S.N. preferredExperience Requirements: - Knowledge of current health care administration principles and concepts- Knowledge of JCAHO accreditation- Understanding of computer systems- Acute care experience- Utilization Review, Discharge Planning or Clinical Case Management experience- Certification in Case Management, desirableMINIMUM REQUIREMENTSLicense/Registration/Certification:- Current and active CA RN license *** This position is being opened as a Non-Training position.
