Experienced Practitioners sought for Complex Disease Management Program
Our innovative practice has a broad range of medical management programs to support our clinicians in caring for their patients. We seek a seasoned clinician to independently manage a panel of referred patients from within our practice to this program that serves those with chronic and complex diseases. The process of care includes and requires diagnostic skill across the spectrum of illness, the ability to integrate that knowledge base with the design and prescription of a treatment plan, assumption of responsibility for managing chronically ill patients, and skills in patient education and counseling while coaching patients to live with illness. Candidates should have a current knowledge base of treatment modalities supporting CDM practice, specifically coronary artery disease / congestive heart failure, diabetes, and related disease management. They shall be well suited to a referral practice and comfortable with internal marketing and promotion of the Complex Chronic Care (CCC) program.Successful candidates will have an ability to collaborate and consult effectively and efficiently with the CCC team, referring PCPs, Specialists, Internal Medicine and Urgent Care clinicians. Their clinical experience of at least two years at the advanced practice level will preferably include practice in ambulatory and/or critical or coronary care hospital-based units.There is some flexibility with schedule.
