Intermountain is recognized as a leading healthcare system and a model for what is right in American medicine.The Health Plans Care Manager will formulate, implement, coordinate, monitor and evaluate strategies for patients and families collaboratively with the member, families and health care team. The Care Manager will develop, document and implement a plan or plans, which will provide the appropriate resources to address social, physical, mental, emotional, spiritual and supportive needs.This position is Monday-Friday 8:00 a.m.-5:00 p.m. Professional Exempt position-Must be currently licensed as a Registered Nurse in the State of Utah.-BSN and Certified Case Manager (CCM) recommended.-Associates Degree Required-A minimum of 3 years of clinical nursing, care management, utilization management, QA, home care, community health or occupational health experience is required.-Experience working with third party payers is desirable with knowledge of products offered by IHC Health Plans preferred.-Written and verbal skills to professionally and effectively communicate with all individuals.-Skills in ICD-9/CPT 4 coding, length of stay guidelines and appropriate levels of care and modalities by diagnosis/procedure in multiple clinical areas required.-Demonstrated computer ability and knowledge to include Windows, word processing and database systems. Must pass PC skills assessment within 30 days or hire.-Be able to work independently and be self-motivated with a positive attitude and flexibility in a rapidly changing environment.
