HOME CARE DIVISION

Position is responsible for making a significant contribution to the improvement in Case Mix Weight and Home Compare patient outcomes through a multi-faceted approach.Licensed professional with experience in completing OASIS documents in a home health care setting. OASIS certification preferred.- Excellent Patient assessment skills.- Ability to teach and role model for colleagues. - Ability to analyze and interpret data.- Demonstrated organizational skills, self directed and independent in fulfilling role obligation. - Advanced computer skills in a Windows environment including Microsoft Word, Excel, and Outlook.

Reporting to the Clinical Manager, plans, coordinates, supervises and/or provides nursing care for patients in the home in order to prevent or postpone institutionalization. Bachelors degree in Nursing and a minimum of one year of nursing experience in Home Care setting required.Current IL RN licensure; Current Illinois drivers license, auto liability insurance and CPR card.
