Description Are you a critical care nurse (ICU, ER, and Recovery Room) with strong clinical skills ready to make a contribution to the Hospital during the daylight hours? We are searching for highly skilled critical care nurses who are ready to leave the direct patient care areas while utilizing their clinical skills. The Case Manager at MCA is responsible for the daily assessment of clinical appropriateness through-out the acute hospital stay. In addition, the Case Manager will be responsible for leading patient care collaboration efforts ( including but not limited to interaction with patients, families, physicians and other allied health professionals) resulting in the patient being placed in the right level of care at the right time. The Case Manager must possess excellent interpersonal and communication skills, a comfort level interacting with physicians as well as the ability to exceed customer expectations The Case Manager performs pre-admission screening for patients. Assists with admission process. Assists with program develoment. Plans, organizes and exeucutes community education/marketing activities. Responsible for performing patient pre-admission assessments and reassessments on a regular basis in accordance with nursing standards of care and with a rehabilitation skilled nursing focus. Responsible for collaboration with patient, family, physician, therapists, nurses and case managers to initiate successful and appropriate admission. Responsible for securing pre-admission authorization with the case manager. Responsible for maintianing physician contact as well as participating in community education activities. Demonstrates knowledge, ability and judgement necessary to perform within the guidelines of disease specific care that includes but not limited to: neuroscience, orthopedic, oncology, cardiology disorders Qualifications Required Job Qualifications include:Current licensure as an RNCurrent Healthcare Provider CPRGraduate of an accredited school of professional nursing. Bachelors of Science in Nursing preferred.3 - 5 years of clinical nursing experience.Knowledge of CMS guidelines and experience working with payor specific guidelines and contractual rules preferred.Customer service abilities including effective listening skills.Critical thinking skills, decisive judgment and the ability to work with minimal supervision meeting firm deadlines.Assertive and persuasive communication skills, action oriented.
