The Nurse Navigator functions as a member of the multidisciplinary team as an advocate and educator for patients from point of entry, through diagnostic studies, diagnosis and treatment plan for cancer. Job duties and responsibilities include:- Coordinates a plan of care including appointments, transportation, education, serving as a liaison throughout the facility and in the community regarding services for this unique population- Develops relationships with radiologists, surgeons, oncologists, radiation therapists, hospitalists, and other members of the interdisciplinary team to care for patients with a diagnosis of cancer- Demonstrates knowledge, skill, and coordination to provide nursing care and guidance to the patient from screening to survivorship- Systematically and continually performs the functions of assessing, planning, implementing and evaluating the care according to the nursing process and Oncology Nursing Society Standards- Navigates and advocates for the patient through the clinical and diagnostic process for breast health and other cancers as applicable- Provides education and information to the patient and family, helping to make the care seamless, continuous and comprehensive- Partners with patient, family and interdisciplinary team and community resources to provide well-coordinated, timely, compassionate, exemplary interdisciplinary care- Communicates with all members of the healthcare team- Initiates and performs ongoing review of policies related to service provided- Supports and participates in health promotion/educational community involvement, life time learning and nursing research- Is a liaison with the American Cancer Society and coordinates related MCP Support Groups- Serves as a resource for community educational events, such as health fairs, screenings, symposiums and lectures as well as staff education- Participate in development and evaluation of professional practice by participating on MCP Cancer Committee, hospital committees, special projects and attendance at staff meetings and town halls- Actively participates in performance improvement activities including ACS National Accreditation Programs Qualifications: Required Job Qualifications include:- Current licensure as an RN- Graduate of an accredited school of professional nursing. BSN required (or plan actively in place to obtain)- National specialty certification preferred- Must have at least 5 years of nursing experience with at least 2 years in Oncology nursing- Customer service abilities including effective listening skills.- Critical thinking skills, decisive judgment and the ability to work with minimal supervision in a fast paced environment.- Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/or equipment up to 50 pounds.
