The Director of Care Management plans, organizes, implements, and directs care coordination activities. He/she is responsible for providing oversight and broad direction and strategic planning to case management, which includes care coordination, utilization review, social services, and discharge planning. The Director of Care Management works closely with the medical staff and MHS leadership while providing synergy with operational leaders by developing ongoing strategies to assure high quality case management outcomes. The Director of Care Management expedites the patient�s progression along the continuum of care to effect timely and appropriate distribution of the patient to the appropriate level of care for all patient populations admitted to MHS. Units reporting to the Director of Care Management include: care managers, nurse navigators, utilization review, social services, and discharge planning.The role of director directs and maintains service line and departmental activity; creating key collaborations and relationship that contribute to success of area of responsibility; incorporates innovation in department/service line; utilizes diagnostic tools to analyze data, trends, and makes recommendations; implements strategic, operational and departmental/service line plans; defines and holds managers responsible for Operating Priorities; is accountable for budget preparation and implementation; selects managers: coaches, mentors, retention officer for direct reports and Level I leaders; engages and collaborates with physicians in capital budgeting & program development; directs and participates in Professional Development Planning Process. Minimum Education: MSN (Masters of Science in Nursing) or MSW (Masters in Social Work). Current RN or Clinical Social Worker license to practice in the State of Colorado, or willing to obtain a Colorado license. Minimum Experience: At least 3 years of recent acute care clinical nursing or MSW experience related to care management required. Experience in utilization review preferred. Experience with medical statistical process and control procedures is required. Must possess excellent written and verbal communication skills and demonstrate the ability to prepare statistical data and report for formal presentations. Successful candidate will have experience running a robust acute hospital care management program.Salary range begins at $95,202
