MetroWest Medical Center is seeking two (2) talented Case Mangers to work 24 hours per week.

The 319-bed regional health care system includes Framingham Union Hospital, Leonard Morse Hospital in Natick and the MetroWest Wellness Center.We are seeking two part-time (24 hours) CASE MANAGERS for our Framingham and Natick campuses. Candidates must have experience in either the Med/Surg department or Psychiatric department as a Staff RN or Case Manager.Under general supervision of the Director of Case Management, is responsible for the management of patients throughout the health care continuum. Facilitates the achievement of quality clinical and cost outcomes. Provides services to patients of all ages. PRINCIPAL DUTIES AND RESPONSIBILITIES: 1. Activates a process for each patient and uses a case management plan in managing care consistent with predetermined financial and quality outcomes, in collaboration with physicians and other healthcare members and payors. 2. Establishes collaborative practices with nursing, physicians, and health care professionals regarding plan of care, variances in plan implementation, and outcomes or expected outcomes. 3. Assesses, gathers information on physical, psychosocial, spiritual functioning, coping abilities, available resources, and support systems. Identifies potential or actual health problems, establishes nursing diagnosis, formulates plan and nursing intervention during inpatient stay, and plans for discharge. Facilitates transition of care to appropriate level of care and facility. 4. Manages and/or delegates care for case load during the patient health care continuum or episode of care. Facilitates communication with all departments. Collaborates for timely outcomes; prevents duplication and fragmentation of services. Assesses educational needs of staff and provides appropriate education. 5. Is responsible for internal and external patient care standards. Identifies and documents incidents that may affect quality of care. Tracks and reports on variances to department. Maintains documentation of case management activities, plans, and outcomes. KNOWLEDGE, EXPERIENCE , AND OTHER REQUIREMENTS: 1. The level of knowledge equivalent to that ordinarily acquired through completion of a BSN. Current licensure to practice nursing in the Commonwealth of Massachusetts. 2. Minimum of 3 to 5 years experience in the specialties listed above, and utilization review or related area. 3. At least three (3) years as an RN WORKING CONDITIONS: Patient care environment. Prolonged standing and walking. JOB RESPONSIBILITIES:1. CLINICAL PRACTICE: Performs all aspects of clinical practice according to standards of care, including, but not limited to: assessment, planning, implementing, evaluation, intervention, organization of work, priority setting, decision making, follow-up, activities of daily living. 2. LEADERSHIP: Delegates tasks to subordinates to ensure efficient and effective work flow. Provides input into hiring and evaluations and participates in the interviewing process. Resolves conflicts. Ensures proper staffing patterns according to established nurse-patient ratios. Assures current and quality nursing practice is maintained through participating in and keeping informed of unit/Hospital committees, staff meetings and other related activities. May serve as charge person. 3. DOCUMENTATION: Maintains thorough and complete documentation according to established legal and Hospital guidelines. Transcribes and inputs physician orders accurately and in a timely manner, using the nurse order entry system. 4. COMMUNICATION: Communicates and collaborates with all health team members, patients, and families effectively. Ensures patient care needs are met in a timely manner and follow through is initiated and/or completed. May act as a patient advocate. 5. EDUCATION: Keeps current with nursing skills, equipment, and unit specific knowledge. Provides patient/family teaching, may act as a preceptor, assists with the professional development of the unit. JOB REQUIREMENTS:-Current RN licensure in Massachusetts-BS Degree in Nursing preferred-A minimum of 2 years of RN experience-Great interpersonal skills, team playerMetroWest Medical Center offers a very competitive pay rate and benefits package.
