Location/ Company: Moffitt Cancer Center (on USF´s campus) Req ID: 5693 Cost Center: 16931 GI Tumor Clinic Employment Status: Full Time FTE: .8 Workdays: Mon-Fri Hours: 8am-5pm, Variable -------------------------------------------------------------------------------- Job Summary: 1 Patient Assessment a Under the direction of a Clinic RN, performs nursing assessment to collect and record relevant medical information including patient needs, symptoms, vital signs and current medications. b Communicates patient data to appropriate members of the health care team. c Collaberates with a Clinic RN for assistance with initial assessment as required. d Documents, clearly and concisely, initial and ongoing patient assessment data (including telephone contacts) in the permanent record. 2 Patient Care a Prepares for Clinic visit by ensuring patient records are present including pertinent test results / films, and previous visit notes. b Delivers Clinic nursing care as prescibed while adhering to polices, procedures and licensure limits and according to age specific standards for the patient (i.e. adult or geriatric). Maintains competence in the provision of care and services to patients of the various ages and abilities served. Adolescents - age 15-18 Young Adults - age 19-30 Mature Adults - age 31-45 Older Adults - age 46-65 Aging Adults - age 66+ c Identifies priorities of care and organizes work flow to deliver safe and efficient patient care. d Provides education based on assessment of the patient’s needs, desire, readiness to learn, and expected outcomes. Utilizes standard Clinic approved education materials. e Provides and plans immediate and future interventions, including referrals to other services and outside agencies. f Interventions are implemented in collaboration with other members of the health care team. Documentation reflects collaborations and interventions. g Accurately and completely documents actions taken during current visit, including referral to other services. Ensures patient’s forms and charge tickets are completed. h Demonstrates proficiency in operating multiple electronic applications used for documentation and obtaining results. Acts as a resource to assist others. i Facilitates continuity of care by communicating treatment plans with patient, other team members and appropriate outside agencies and/or programs. j Acts as an advocate to help patients achieve their desired outcomes. k Responds promptly and according to procedures in emergency situations 3 Facilitiates Patient Flow in the Clinic a Demonstrates strong organizational skills in maintaining a smooth flow of patient activity, minimizes delays by keeping patients ready for physician. b Facilitates the flow of patient materials after the care provider has visited with the patientand documents evaluation to nursing interventions during initial, follow-up visits and telephone contacts. c Communicates patient delays to appropriate staff members and assist with problem solving. d Problem solves to locate and obtain missing or detained materials as needed. 4 Organizational Skills a Demonstrates an ability to manage own patient assignment and prioritizes work load to assist others. b Appropriately delegates to co-workers based on job descriptions and with consideration of the individual skill level and unit needs. c Ensures appropriate supplies and equipment are available in Clinic to support the care required for all typically scheduled patients. 5 Interacts directly with patients, managers and other department personnel a Personalizes the care by establishing a congenial rapport with the patient. b Displays a professional appearance and impression, adhering to dress codes guidelines. c Receives phone calls from internal and external customers. Triages calls appropriately. d Carries pager during work hours. Responds to pages within 15 minutes. e Exercises independent judgement to facilitate optimal patient-care. f Informs appropriate areas and personnel of concerns, changes, or delays. g Acts as a resource to patients, addressing questions and concerns within the realm of responsibility. Directs question appropriately. h Displays a professional appearance and impression, adhering to dress code guidelines. 6 Maintains an updated knowledge of the clinic; maintains and enhances skills a Reads and applies departmental and institutional communications and updates. b Attends and participates in at least 10 staff meeting per year or obtains information presented (if absent) by reading/initialing the minutes. c Initiates discussion regarding unit or department issues. Report Committee results. d Participates in nursing research by implementing patient care in accordance with specified Moffitt IRB approved protocol. e Demonstrates active role in at least one of the following areas through participation and presentation of progress reports: • Moffitt Clinic committee, task force or project. • Cancer Center committee or task force. f Department specific committee, task force or project. Minimum of 16 relevant contact hours earned during the past year. g Seeks clinical consultation with RN III, RN IV, Clinical Nurse Specialist and Clinic Operations Manager.
