Job Title: LVN II Location: Jefferson Building Full/Part: Full-Time Days - 8hr Shift: Responsibilities 1. Advocates for patients/families to ensure patient rights and promote integration of personal, cultural, social and spiritual preferences into care provision. 2. Demonstrates stewardship of resources such as time, materials and persons while maintaining excellence in the provision of care. 3. Utilizes basic critical thinking skills and the nursing process to provide and coordinate patient care on patients with predictable outcomes. Documents in compliance with network standards. 4. Educates patients/families concerning disease, treatment plan including self care post discharge, holistic health needs, resources available to patient/family, rehabilitation and follow-up care. 5. Functions as an effective team member with health care team, patient and families by using basic communication and team building skills such as patient interview, dialogue, assertiveness, collaboration and conflict resolution. 6. Supports professional standards of care delivery by (a) functioning within scope of LVN title act and SETON Healthcare Network policy and procedures, (b) assuming responsibility for own learning needs and (c) utilizing appropriate resources (RN and other healthcare members) as needed 7. Participates as a staff educator in the development, implementation and evaluation of educational processes that assist others along the novice to expert continuum. Minimum Qualifications Six (6) months experience as an LVN in an acute care setting is required. Current Licensed Vocational Nurse (LVN) license to practice in the state of Texas is required. Skills Completion of LVN II competencies for unit, specialty and network is required.
