SMC 5 - Gen Medical Surgical Responsibilities CA I: 1. Performs basic nursing tasks under the delegation and supervision of the RN and according to each patient/clients individualized plan of care. Nursing tasks are provided with courtesy and sensitivity to the needs of the clients. Examples include, but are not limited to, the following: a. Collecting, reporting and documentation of data including, but not limited to: Vital signs, height, weight, intake and output, oximetry and comfort level; Specimens for diagnostic study - urine, stool, sputum and blood via finger stick for blood glucose testing; Changes to baseline data established by the RN; and client or family comments relating to the clients care and behaviors related to the plan of care b. Provides basic materials management support as needed including, but not limited to: a. Stocking and obtaining unit supplies and equipment b. Identifying, labeling and reporting malfunctioning equipment 5. Provides basic housekeeping duties as needed including, but not limited to: a. Assisting in maintaining a clean, safe and orderly environment for patients and staff b. Assisting in preparing and stripping rooms as needed. CA II: 1. Obtains certification and maintains competency to perform venipuncture for phlebotomy 2. Assists with clean and/or sterile dressing changes in departments where this task is frequently performed after completing SETON”s certification and competency check-off. 3. After completing SETON”s certification and competency check-off, performs applicable tasks which may include: (a) urinary catheterization, (b) electronic monitor observation, (c) removal of staples and simple sutures, (d) removal of peripheral IV catheters. 4. Reinforces health teaching that is planned and/or provided by the registered nurse. 5. Performs clerical duties which may include, but are not limited to: A. Entering orders and messages into and retrieving information from all appropriate computer systems. B. Transcribing physician orders with verification by RN. C. Ordering unit supplies (lab supplies, nourishments, office supplies, chart forms) Minimum Qualifications CA I: High school diploma (or equivalent) OR one (1) year of related experience OR certification as a Certified Nurse Aide (CNA) OR participant in formal health care training program is required. Completion of required training within one month of hire is required. Within one month of hire, must obtain CPR certification through the American Heart Association. CA II: High school diploma or equivalent is required. Six (6) months of experience for external candidates in a related position, completion of one semester of nursing clinicals, or completion of a GAIN approved course is required. Internal candidates must have successfully completed Clinical Assistant I and II competency checklist. Department specific certification may be required. Performs other assigned duties which are directed and supervised by the RN. 2. Maintains competency and required certification such as CPR and blood glucose testing via finger stick. 3. Maintains educational records including competency, mandatory and regulatory requirements. 4. Participates in meetings and committees as required.

Responsibilities CA I: 1. Performs basic nursing tasks under the delegation and supervision of the RN and according to each patient/clients individualized plan of care. Nursing tasks are provided with courtesy and sensitivity to the needs of the clients. Examples include, but are not limited to, the following: a. Collecting, reporting and documentation of data including, but not limited to: Vital signs, height, weight, intake and output, oximetry and comfort level; Specimens for diagnostic study - urine, stool, sputum and blood via finger stick for blood glucose testing; Changes to baseline data established by the RN; and client or family comments relating to the clients care and behaviors related to the plan of care b. Ambulation, positioning and turning c. Assisting with admissions, discharges, transfers and transportation of patients d. Activities of daily living to include personal hygiene, skin care, bed making and elimination including vaginal irrigations, cleansing enemas, ostomy care and oral suctioning e. Passing and collecting meal trays/menus and providing food and fluids as ordered f. Socialization activities 2. Maintains confidentiality for written, verbal and computerized information. 3. Provides basic communication and clerical support for unit functions including, but not limited to: a. Assisting with answering and responding to call lights and telephone calls b. Utilizing equipment necessary for communication - intercom system, paging system, fax machine, pneumatic tube system c. Assembling and maintaining departmental and patient records 4. Provides basic materials management support as needed including, but not limited to: a. Stocking and obtaining unit supplies and equipment b. Identifying, labeling and reporting malfunctioning equipment 5. Provides basic housekeeping duties as needed including, but not limited to: a. Assisting in maintaining a clean, safe and orderly environment for patients and staff b. Assisting in preparing and stripping rooms as needed. CA II: 1. Obtains certification and maintains competency to perform venipuncture for phlebotomy 2. Assists with clean and/or sterile dressing changes in departments where this task is frequently performed after completing SETON’s certification and competency check-off. 3. After completing SETON’s certification and competency check-off, performs applicable tasks which may include: (a) urinary catheterization, (b) electronic monitor observation, (c) removal of staples and simple sutures, (d) removal of peripheral IV catheters. 4. Reinforces health teaching that is planned and/or provided by the registered nurse. 5. Performs clerical duties which may include, but are not limited to: A. Entering orders and messages into and retrieving information from all appropriate computer systems. B. Transcribing physician orders with verification by RN. C. Ordering unit supplies (lab supplies, nourishments, office supplies, chart forms) Minimum Qualifications CA I: High school diploma (or equivalent) OR one (1) year of related experience OR certification as a Certified Nurse Aide (CNA) OR participant in formal health care training program is required. Completion of required training within one month of hire is required. Within one month of hire, must obtain CPR certification through the American Heart Association. CA II: High school diploma or equivalent is required. Six (6) months of experience for external candidates in a related position, completion of one semester of nursing clinicals, or completion of a GAIN approved course is required. Internal candidates must have successfully completed Clinical Assistant I and II competency checklist. Department specific certification may be required. Within one month of hire, must obtain CPR certification through the American Heart Association.
