POSITION REQUIREMENTS The Palliative Care Program consists of an interdisciplinary team, including nursing, pharmacy, spiritual care, case management, social work, pain management, and physician leadership. The team provides holistic compassionate care that embraces the patient and family as a unit and recognizes cultural, ethnic, and spiritual differences. The teams aim is to relieve suffering and improve quality of life during any state in the course of a life-threatening illness, from diagnosis through the end of life and bereavement. JOB DUTIES: Performs comprehensive appraisal of patients for pain management. Collaborates with the Medical Director of the Pain and Palliative Division and the Lead Nurse practitioner. Assumes clinical responsibility for tasks performed within the Nurse Practitioners scope of practice including taking patients history, performing physical examination, ordering and interpreting diagnostic tests, and prescribing appropriate therapy. Evaluates outcomes of direct patient care and consults with other specialists as required. Acts as consultant for various patient referrals. Actively participates in multidisciplinary treatment planning. Provides assessment and follows established medical standards, procedures and practices to develop a treatment plan for the patient. Performs quality improvement activities as assigned. Provides education to patients, families, community, and colleagues using various teaching strategies. Utilizes special educational methods that are age appropriate. Maintains accurate documentation in the medical record including problem list and charges for services and gives patient care direction to nursing and other staff. Other duties as assigned REQUIREMENTS:Registered and certified in AZ as an ADULT Nurse Practitioner. ACLS certified. One year experience as a nurse practitioner in pain management and >5 years experience in pain management or palliative care nursing specialist position preferred. Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. Good organizational skills.
