Detailed Position Description Identify, assess and coordinate the care of select Hematological Malignancy and Blood and Marrow Transplant adult patient population comprehensive family-centered discharge planning in an efficient and cost effective manner. Identify patient with complex healthcare/rehab needs. Assess individual patient and patient populations to identify care/education/rehab needs and develops, implements and evaluates programs to meet these needs and coordinate patient care/collaborates with others. Practices utilization management. Patient documentation.Actively participates in the quality improvement process. Involved in learning regarding trends in case management. Data analysis. Initiates and maintains positive relationships with patients/customers. Initiates and maintains positive relationships with co-workers. Takes responsibility for self-development and supports a learning environment. Displays commitment to the mission of the hospital and its values. Position Requirements Demonstrated competence in identified area of expertise. Demonstrates strong interpersonal and leadership skills. Demonstrates a professional demeanor when responding to patients. Uses a wide variety of technical equipment to meet education and patient care needs (eg. Stethoscope, cardio-respiratory monitor). Education Requirements Nursing Doctorate or Master of Science in Nursing with Advanced Practice Certification License/Certification Requirements Registered Nurse with current license to practice in the State of Ohio;Certified as a Nurse Practitioner by a certifying body approved by the Ohio Board of Nursing;Valid and active Certificate of Authority to practice in the State of Ohio by the Ohio Board of Nursing;Valid and active Certificate to Prescribe in the State of Ohio by the Ohio Board Nursing (when applicable) Additional Details This position will be focused on the Hematological Malignancy and Blood and Marrow Transplant adult patient population.
