Description * Deliver and facilitate coordination of palliative care services to patients, families and caregivers across all patient care settings (inpatient, outpatient, LTC)* Provide education for WDH staff and healthcare providers in our community (focus on LTC facilities)* Build and strengthen effective relationships with referring clinicians to improve the culture of collaboration among care providers and WDH staff* Enhance communication with the PC and LTC facility and Hospice Medical Directors* Proactively identify opportunities to improve the patient and family experience of care* Demonstrate excellence in leadership skills and professional performance Requirements -Graduation from accredited Nurse Practitioner Program or with certification and/or experience in Geriatrics, Palliative Care or Hospice -BSN is desired-NH License Posted Hours 40 Shift Days
