Our Somerville and Post Office Square practice locations offers multiple specialty services including Orthopedics, Urology, Dermatology and Neurology and has a surgical suite that provides conscious sedation for special procedures, and a range of diagnostic imaging capabilities. A full-time Analyst works on site supporting users of our EMR/Epic system. The practices are conveniently located near the MBTA.The Nurse Practitioner sees roughly three patients per hour, and must work collaboratively with other clinicians delivering comprehensive care and must possess the ability to assess clinical problems, determine priorities of care, design and implement a treatment plan and seek and utilize consultation appropriately. The NP must have graduated from a masters level state and NLN approved NP program and possess state licensure to practice in the expanded role. Spanish, Portuguese and Haitian Creole language skills are a plus. Experience - Three years of advanced practice and ambulatory experience is preferred, and experience in chronic disease management is strongly preferred. Successful candidates will also have great communication skills with patients and colleagues.

The individual serves as a clinical resource and educator for the patients, their families, the community and Saint Agnes Hospital associates. The individual integrates the components of clinician, teacher, individual consultant and researcher in the performance of duties. The nurse practitioner utilizes the nursing process while supervising patient consultation and treatment. The individual must possess the ability to make independent judgments within the scope of nursing practice. The individual demonstrates knowledge and skills necessary to provide care appropriate to the age of patients served in the assigned area. The individual interprets appropriate information needed to identify each patients requirements relative to his/her age specific needs and provides care needed as described in the hospital and departments policies and procedures. The individual works in conjunction with physicians, nurses, therapists, life coaches and other disciplines to assure an interdisciplinary approach to patient care. The individual demonstrates expertise regarding policies, protocols and regulations applying that knowledge to ensure the smooth flow of information to and from the department. The individual assists in the training of new associates. Carries out responsibilities in accordance with Ascension Health Core Values, St. Agnes policies/procedures and applicable civil laws. Is accountable for efficient use of resources and support of the St. Agnes mission. REQUIRED EDUCATION: Masters of Science in the family or adult primary care nurse practitioner tract with board certification.
