Qualifications -Required License/Certification - BLS / CPR -Required License/Certification - Advance Practice Nurse with prescriptive Authority or eligibility to receive prescriptive authority. -Required License/Certification- Certified Family Nurse Practitioner or Pediatric Nurse Practitioner within 6 months of employment -Required License/Certification - Registered Nurse -Required Education- Master of Science in Nursing: Family Nurse Practitioner or Pediatric Nurse Practitioner Physical Requirements Ability to Perform Essential Functions of the Job Job Overview The Advanced Practice Nurse utilizes nursing arts and sciences, child development, education, psychosocial and cultural theories to facilitate the delivery of comprehensive nursing care to patient and families. The APN works independently and as a member of a multidisciplinary team to provide quality patient management, education and continuity of care. Major responsibilities included but are not limited to demonstrated leadership in the following: clinical practice, consultation, education, research/publication, fiscal responsibility, and professional development. A primary responsibility of this position will be Care Coordination for the patient, family and multidisciplinary team for a safe, quality discharge from the acute care setting. Must have clear knowledge of patient flow and services the SMSC provides to the patients, families, physicians and ancillary services. Must demonstrate exceptional Standards of Behavior- Quality and Safety, Communication, Ownership, Relationships, Professionalism and Teamwork.
